Health Systems Information Services created a SharePoint template that provides web site owners with an easy way to create web forms, customize information collected in the form, and access/export the responses. This can all be done without any programming.
It is helpful to design your form before you get to SharePoint — Know what information you need to collect (name, email, school/college, profession, etc.). Remember that less is more. The shorter your form is the more likely the visitor will complete the form. Think through the following questions:
